If you were left in doubt about what happened at the end of the fourth episode of Moon Knight, the comic series by Jeff Lemire could explain in detail.
The Moon Knight series is in the final stretch of its first season, and episode 4 of this new proposal from Marvel Studios presented us with an unexpected twist in its plot, a situation that is explained thanks to the comics authored by Jeff Lemire, Greg Smallwood and Jordie Bellaire.

After Arthur Harrow shoots Steven Grant, Marc Spector wakes up in a psychiatric hospital where familiar faces begin to parade, such as Donna, Steven Grant’s boss at the London museum, or even Hagrow himself, who is the local psychiatrist.
Marc Spector finds a sarcophagus in this place where Steven Grant appears, his other personality, with whom he interacts, and to whom he proposes to escape from the place.
After that, Grant and Spector find a new sarcophagus, which they decide not to open, despite the fact that there is obviously someone in that coffin. The action ends when Grant and Spector meet the Egyptian goddess Tawaret, whose form is that of a hippopotamus.
The inspiration behind the psychiatric hospital
The end of the fourth episode of Moon Knight is based on the run by Jeff Lemire, published in the first five issues of Moon Knight (April, 2016), where after a lifetime fighting crime, Marc Spector wakes up in an insane asylum, without powers and with a complete medical file.
It turns out that this asylum is known as The Othervoid (the other void), a dimension inhabited by Egyptian gods who sought to get out of there, but this was not possible thanks to Mon Knight.
Given this, Gods like Ammit transported the mind of Marc Spector, and taking advantage of his dissociative identity problem, they filled his head with various lies, using familiar faces for the ex-mercenary and avatar of Khonshu.
The different personalities of Marc Spector can interact in The Othervoid, since these alter egos live as separate entities with consciousness, which could escape from this Other Void through the Egyptian God Annubis, who would be a kind of ferryman, who will demand payment of a soul
Oscar Isaac stars in Moon Knight, with a cast consisting of Ethan Hawke and May Calamawy. Mohamed Diab and the team of Justin Benson and Aaron Moorhead directed all six episodes. Jeremy Slater is the showrunner for the first season.
