Requirements:
The following documents should be sent to the aforementioned email in PDF version:
- Format SIAB-VIDA request filled out and signed. If this is the case, write down the name and address of the representative or person promoting on your behalf.
- Official identification of the applicant.
- Death certificate of the deceased.
- If the procedure is carried out through a proxy, attach the document stating the power of attorney or representation.
